PATIENT INFORMATION NOTICE

 COVID Restriction Update

Throughout the pandemic we have strived to make the Surgery as safe as possible for all our patients and staff. We will continue to do so going forward.
From the 19th July, nothing will change at the surgery - patients will need to still wear a face covering and socially distance and staff will continue to wear PPE as agreed with Public Health England.
The number of patients allowed into the waiting area will also continue to be restricted. We do understand that this can be frustrating but we know that when people come to us they are sick and vulnerable so we feel it would be wrong to not take these small precautions to protect us all.
COVID has not gone away.
In terms of opening up, as many of you know, we have never been shut and in fact have consulted with much higher numbers of patients than we did pre-COVID (face to face, by phone, and by video), not counting vaccinating 90% of the adult population twice in the last 6 months. We have come so far in trying to minimise the impact of COVID and we are sure you understand that we need to keep the surgery as safe as possible for ALL patients and staff. One positive case of COVID within the surgery could wipe out our entire workforce for 10 days, and I'm sure you will agree, maintaining restrictions is a small price to pay in order that we continue with a full range of medical services, and keep our doors open.
Many Thanks

BUILDING WORK ANNOUNCEMENT
As from the 19th of June building work will begin at the Surgery. A new entrance is being built to replace the existing porch. The build is expected to last 6-8 weeks.
During this time there will be considerable disruption for the patients. We will ensure you have safe and easy access to the surgery. This will mean you will now pick up your prescriptions from the front waiting room window which will be clearly marked. To accommodate this we would still ask that you only collect prescriptions between 11am -1pm and 3pm-5.30pm.
If you have an appointment or need to speak to a receptionist please use the right hand side of the building where you will find an intercom linked to reception. Please be mindful of social distancing on this path.
Please only turn up at the surgery for an appointment at your allotted time as we have limited space in the waiting room due to COVID restrictions and the building works. Please remember to wear a mask before entering the building.
Disabled parking- parking will still be available although limited. If you require disabled access please use the right hand corner in the front of the building.

REGISTERING FOR YOUR TYPE 1 OPT-OUT

If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out.

This collection will start on 1 July 2021 so if you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out.

The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.

If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.
To register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out) you can download the form here and then return it to the surgery.

More information about this matter can be viewed on the NHS Digital website by clicking the link here.

Information about "Extended Access"

Under the Extended Hours Primary Care Services programme, there is a requirement for patients to be able to access 1½ hours additional appointments after 6.30pm each week day and an effective weekend service based on the local needs of the area. All practices must be able to direct patients to the service and offer a choice of evening or weekend appointments on an equal footing to core hours appointments.

We are pleased to announce that appointments are now available between 4pm and 8pm weekdays and clinics on a Saturday and Sunday at the following Hubs. These will be run by a team of GP Federation Locum GPs who will have access to your medical records following your consent to share. Please ask our Receptionists about this service as these appointments can only be booked via this Practice.
